A Protruding Belly Is Not Just a Cosmetic Issue: The Hidden Dangers of Visceral Fat

Visceral fat surrounding the internal organs in the abdomen is far more dangerous than fat under the skin. This is not about aesthetics — it's about the risk of heart attack, stroke, and diabetes that can be predicted and prevented.

Patients often ask: "Doctor, I'm not that fat, but my belly is big. Is that dangerous?" My answer is always: yes, possibly even more dangerous than someone who is evenly overweight without a protruding belly. This is why understanding visceral fat is essential health knowledge for every adult.

Two Very Different Types of Body Fat

Not all fat is created equal. There are two main types:

  • Subcutaneous fat: Fat under the skin — the kind you can "pinch" on your arms, thighs, or buttocks. Metabolically, it is relatively harmless.
  • Visceral fat: Fat surrounding the organs in the abdominal cavity (liver, pancreas, intestines). It can't be pinched because it's on the inside. This is the dangerous kind.

Visceral fat is not "idle" fat — it is metabolically active tissue that continuously releases inflammatory substances (pro-inflammatory cytokines) and free fatty acids directly into the bloodstream towards the liver.

Why Is Visceral Fat So Dangerous?

Here is the chain of mechanisms that occurs:

  1. Visceral fat releases free fatty acids directly into the portal vein towards the liver
  2. The liver becomes "flooded" with fatty acids → fatty liver, increased triglyceride production
  3. Insulin resistance develops in the liver → pushing the pancreas to work harder → prediabetes
  4. Chronic systemic inflammation from pro-inflammatory cytokines → damages blood vessel walls → atherosclerosis → risk of heart attack and stroke
  5. Blood pressure tends to rise due to increased renin-angiotensin system activity

Metabolic Syndrome Criteria: Do You Qualify?

Metabolic syndrome is a cluster of five conditions that often appear together as a result of visceral fat and insulin resistance. It is diagnosed when at least 3 of the following 5 criteria are met:

Why Do Diets Often Fail Without a Medical Approach?

Many patients have tried various diet programmes but the belly fat remains stubborn. There are medical reasons that explain this:

  • The hormones ghrelin (hunger) and leptin (satiety) are disrupted by chronic obesity — the brain no longer reads satiety signals correctly
  • Chronic cortisol (the stress hormone) specifically deposits fat in the abdominal area
  • Persistently high insulin (due to insulin resistance) inhibits lipolysis — the body cannot burn stored fat
  • Sleep deprivation significantly disrupts the balance of metabolic hormones

  • A complete metabolic panel (lipid profile, liver and kidney function, thyroid hormones, blood glucose, uric acid)

  • Evaluation of other possible hormonal causes (Cushing's, PCOS, hypothyroidism)
  • A measurable treatment plan: lifestyle changes + medication when indicated + regular monitoring

Cholesterol and Uric Acid Myths That Must Be Corrected

Myth: "High cholesterol must be from eating too much meat and eggs."
Fact: 80% of cholesterol is produced by the liver itself (endogenous), not from food. Genetic factors and insulin resistance are far more decisive for cholesterol levels than daily food intake in most patients.

Myth: "With gout, you can't eat anything."
Fact: Purines do contribute, but fructose (fruit sugar and table sugar) and alcohol are often more significant causes than animal protein. An overly strict diet can actually worsen the condition if not managed properly.
